What condition can cause the following symptoms?

I am a healthy, 32 year old male with no history of medical problems. For the past few years, I have had constant heartburn. I don't mean it recurs a lot; I mean it is always there unless I take an acid reducer on a daily basis. As soon as I skip a dose or two, it is back with a vengeance. It is very painful and causes me to be sick occasionally. I also have a sharp pain that feels like someone is sticking an ice pick through me on my upper left abdomen/lower chest area. It sometimes feels like it's in my upper left back and shoulder blade too. What the heck is wrong with me? Any ideas??
